I own the company and the truck so I can make the move and price very simple.
We can pack up items for additional price.
I own 2 20'FT box trucks. I'm insured and have been in the moving business for 17 years and decided to start my own.
I bought a U-Haul and a Penske truck and been going at it ever since.
We do out of state moves as well. I can give you a rough estimate on packing from pictures. We also do cleaning of apartments as well after we move you out. Doesn't matter if it's house or apartment or how many rooms etc. It's $1000 initially and $400 each truck load after. I can usually get a 3bdrm in one load.

$1000 for every '20ft truck which includes 60 miles.
Additional truck load is $400.
More than 60 miles is $2.50/mile. Mileage is adjusted for long haul moves.
Packing items is additional price and depends on size of box,how many boxes,estimated times to pack etc. usually no more $150-$250 very rare if it's ever been higher than that.
Basements are included in the price.
Attics are an additional fee and vary from $150-$250.
Doesn't matter what floor your on as long as you have a elevator. Additional fees may apply depending on flights of stairs if no elevator available.
