Longitudinal Care Manager (RN)
Location: Remote (Must be able to travel to NYC quarterly)
Type: Temp to Perm
Pay: $61–$63/hr
Schedule: Monday to Friday, 8:30 AM–4:30 PM OR Per Diem!

Licensure Requirement: Must hold an active New York State RN license (can reside out-of-state)
Position Summary
The Longitudinal Care Manager provides ongoing care coordination and management for patients over time, ensuring continuity of care, optimal health outcomes, and appropriate utilization of services. This role supports patients across the continuum of care—following initial assessments—through regular monitoring, plan adjustments, and collaboration with multidisciplinary teams.
Responsibilities
Manage a caseload of patients longitudinally, tracking health status and care needs
Update and adjust individualized care plans based on patient condition and goals
Coordinate with physicians, specialists, and community resources for comprehensive care delivery
Conduct regular patient outreach via phone or telehealth to monitor health and reinforce care plans
Identify barriers to care and implement strategies to address medical, behavioral, or social needs
Document all interactions and care updates accurately in the electronic health record (EHR)
Facilitate transitions between levels of care (initial, palliative, hospice) as appropriate
Advocate for patients and their families across the care journey
Qualifications
Active New York State RN license
Experience in case management, care coordination, or similar clinical roles
Knowledge of chronic disease management and utilization management
Familiarity with Medicare, Medicaid, and D-SNP programs
Excellent communication, organizational, and problem-solving skills
Ability to work independently and manage multiple patients remotely
Preferred Qualifications
Experience with telephonic or remote care management
Familiarity with EHR systems and care management platforms
Prior experience across the full continuum of care (initial, longitudinal, palliative, hospice)
