What You'll Do:
✅ Conduct client interviews, assessments, and new client intakes
✅ Develop individualized service plans to help clients achieve their goals
✅ Connect clients with housing, public benefits, employment, legal, and community resources
✅ Collaborate with landlords and clients to address housing and tenancy concerns
✅ Assist with benefit applications, entitlements, and emergency financial assistance
✅ Provide ongoing case management, follow-ups, and progress tracking
✅ Maintain accurate, audit-ready documentation in paper and electronic records
